Oregon State Student Health Survey Logo

Our 6th and 8th grade students will be asked to take the Oregon's Student Health Survey the first week of March. The survey is anonymous and is a key part of statewide efforts to help local schools and communities ensure that all Oregon youth are healthy and successful. The survey will take place during Advisory on March 2nd & 3rd. 

The survey aims to address student health and safety, mental and behavioral health, school climate and culture and the impact of COVID-19. 

The School district, as well as the Union County Safe Communities Coalition depend on this data to help inform them on where to focus efforts and services that will help to ensure our students, families and communities are safe, happy, and healthy.  

We look forward to your students participation in this survey, however, if you would like to opt them out, please visit our LMS website and click on the Oregon Student Health Survey. Here you will find a form you can print, sign and send back with your student no later than March 1st to be turned into the main office. It is also a place where you can preview the questions that will be asked, if interested. Your student can also pick up an opt out form from the main office, for your convenience.
